Online Forms

You can access additional TRA forms by viewing your member account online.

Choose member login,

Register if you haven’t done so already, and

Enter your User ID and password.

After accessing your online account, forms can be found under the Applications/Forms section of the Member Navigation Screen.

Complete Online PDF	Minnesota State AND Federal Withholding/Change Certificate for Pension or Annuity Payments (TRA-4900) We suggest you complete your tax withholding request by logging in to your TRA member account. Federal tax withholding designations must be based on marital status and number of allowances. Designation of a flat dollar amount will not be accepted. Marital status for both Federal and State tax withholdings must be the same. State tax allowances must be lower or equal to your Federal tax allowances. Apply Online	Refund Application (TRA-7000) You may apply for a refund by logging in to your TRA member account.
